Our full service brokerage serves you by locating the freight, arranging pick up and delivery appointments and
handling all aspects of the billing collection process. For you the carrier, this means money in your pocket
faster and less hassle in arranging loads.
To qualify, you must meet our Carrier Requirements as follows:
1.
Provide us with a copy of your current Contract Authority Certificate. Also any additional authorities you may hold, for example Intrastate or Canadian.
2.
Provide us with an ORIGINAL CERTIFICATE OF INSURANCE listing Nationwide Transportation, Inc. as the Certificate Holder. We require certificates on both your Auto Liability Combined Single Limit (CSL) and cargo coverage. Minimums are $1,000,000 Liability and $100,000 Cargo. The certificate must also show your deductible amount.
3.
An ORIGINAL CERTIFICATE OF WORKMAN’S COMP INSURANCE Coverage.
4.
A copy of your satisfactory DOT safety rating.
5.
A signed contract between you, the carrier, and Nationwide Transportation, Inc. along with a W-9 form.
